Community Care is the help and support we provide to adults and those who care for them, so that people can live as independently as possible in the community.

We can provide support to people in their own homes or in other care settings should that be necessary and also offer support to carers.
It is important that we recognise people's abilities and the optimum level of independence through rehab and reablement. Some people may only need support for a short time others may require ongoing assistance with care. The level and type of support will be based on an assessment of need.
