Community Health and Care Partnership (CHCP) is a Partnership between West Dunbartonshire Council and NHS Greater Glasgow and Clyde  and brings together both NHS and Local Authority responsibilities for community-based health and social care services within a single, integrated structure.

 

It's creation brings tremendous opportunities to continue to develop high quality health care and social care service, as well as provide strong leadership to improve health and well being more broadly across all the communities of West Dunbartonshire.

 

The CHCP came into being on the 1st October 2010, and will build on the strong local track record of joint working between the staff and services of West Dunbartonshire Council and NHS Greater Glasgow and Clyde.

 

The role, responsibilities and accountabilities of the CHCP are set out within the Scheme of Establishment that has been agreed by both NHS Greater Glasgow and Clyde Health Board and West Dunbartonshire Council.
